0480(1) - Provider does not have and follow a transportation and off-site activity policy. Current written parent authorization is not adequate for travel that is happening. Parents are not given adequate notice to this travel. 0480(2) - Attendance observed to not be taken that assures all children are accounted for at the end of travel and when exiting vehicle. 0480(3) - Current insurance policy is not sufficient and does not cover all drivers/vehicles being used.
